APPOINTMENTS STILL AVAILABLE for the Covid Vaccination Clinic Tuesday, April 13th in cooperation with Caring Wellness Pharmacy.  The Moderna vaccine, will be provided by appointment only.  To select an appointment please select a time from the chart below. 

HIT REFRESH on your browser if you don't see many openings.  We have 12 patients allowed for each 15-minute time slot. When 12 appointments are booked, it will not allow any more appointments for that time.  As of Monday 9:00am  there are about 100 appointments still available.  Pre-registration will close at 2:00pm today. After that time, we will have a waiting list.

Vaccine appointments are ONLY available for Individuals Aged 18+ or Individuals with Pre-existing Conditions (Letter from your Doctor) or Comorbidities (Letter from your Doctor).  

An appointment confirmation email will be sent after you schedule your appointment. To shorten your wait time, please be sure to fill out the necessary paperwork prior to your appointment: confirmation of New York State’s COVID-19 Vaccination form and the COVID-19 vaccine consent form. Please bring a government-issued photo ID to your appointment, your Red-White and Blue Medicare card, and/or any other insurance card to your appointment (Including the insurance card you take to your doctor’s office and pharmacy). For those individuals who are unable to complete the paperwork online, there will be forms available at the clinic.

The vaccinations will be performed at the Clifton Park Senior Community Center on the Clifton Common.  The address is 6 Clifton Common Court.  Please arrive five minutes prior to the time of your appointment.  Town staff will greet participants as you enter the facility.
